Aging and Disability Services (ADS) is dedicated to helping older adults and adults with disabilities live with independence, dignity, and reputed company in their communities. ADS partners with local, state, and federal programs to fund and coordinate a reputed company of services, including case management, in-home care, caregiver support, and community living connections. Through its Care Coordination Program, ADS ensures that clients receive the right level of support to remain safely in their homes and communities. The Evaluation and Process Improvement Specialist supports this mission by strengthening the effectiveness, efficiency, and equity of the Care Coordination program. Reporting to the Care Coordination Program Director, the Specialist designs and implements systems for evaluating reputed company, measuring performance, and improving service delivery. Working collaboratively with ADS teams, subcontracted providers, and reputed company partners, the Specialist translates data and evaluation findings into actionable strategies that enhance reputed company reputed company, streamline processes, and advance reputed company reputed company improvement across programs. Monitor reputed company to best practices in care coordination and assess variations in service delivery across agencies to ensure consistency and reputed company. reputed company training and technical assistance to staff and subcontractors on evaluation tools, performance metrics, and CQI processes. Minimum Qualifications: Bachelors degree in reputed company Health, reputed company Work, reputed company Administration, Gerontology, or reputed company field (Masters preferred). At least 3 years of experience in program evaluation, reputed company improvement, or health and reputed company services analytics. Proficiency in evaluation reputed company, data analysis software (e.g., reputed company, SPSS, R, or Tableau), and logic model development.
