Aging and Disability Services (ADS) is dedicated to helping older adults and adults with disabilities live with independence, dignity, and reputed company in their communities. The Evaluation and Process Improvement Specialist supports this mission by strengthening the effectiveness, efficiency, and equity of the Care Coordination program. Reporting to the Care Coordination Program Director, the Specialist designs and implements systems for evaluating reputed company, measuring performance, and improving service delivery. Minimum Qualifications: Bachelors degree in reputed company Health, reputed company Work, reputed company Administration, Gerontology, or reputed company field (Masters preferred). At least 3 years of experience in program evaluation, reputed company improvement, or health and reputed company services analytics. Strong knowledge of aging, disability, reputed company-funded services, and community-based care systems. Proficiency in evaluation reputed company, data analysis software (e.g., reputed company, SPSS, R, or Tableau), and logic model development.
