The key responsibilities of the job may include: Ability to review the patients longitudinal medical record & triage potential conditions based on symptoms, medications, labs, clinical notes, social & family history among other data points. Understanding value-based healthcare in the US particularly key concepts associated with patient risk, major conditions and comorbidities driving healthcare utilisation and consumption, and key shifts in value based care. Conducting deep and thorough evaluation(s) of patient documents to identify missed diagnoses, gaps in care and/or other evidence of risks, diseases and occurrences that could meaningfully increase the patients risks for a poor outcome .
