Medical Coding- EM OP

Key Responsibilities Review and analyze outpatient medical records, physician notes, and clinical documentation. Assign accurate E/M, ICD-10-CM, CPT, and HCPCS codes for outpatient encounters. Ensure compliance with AMA, CMS, AHIMA, AAPC, and client-specific coding guidelines. Identify documentation deficiencies and communicate queries when required. Maintain coding quality and productivity targets as defined by the organization. Participate in internal audits and quality improvement initiatives. Stay updated with coding guideline changes and regulatory updates. Support denial management and coding-related appeals when necessary. Maintain confidentiality and HIPAA compliance while handling patient records. Required Skills Strong knowledge of Outpatient E/M coding guidelines. Expertise in ICD-10-CM, CPT, and HCPCS coding systems. Understanding of medical terminology, anatomy, physiology, and disease processes. Ability to interpret physician documentation accurately. Good analytical, communication, and problem-solving skills. Familiarity with EMR/EHR systems and coding tools. Qualifications Graduate in Life Sciences, Nursing, Pharmacy, or related healthcare field. Certified coder preferred CPC (Certified Professional Coder) Experience Requirements 1-5 years of medical coding experience. At least 1 year of experience in E/M Outpatient coding preferred. Experience working with US healthcare clients is an advantage. .
