Service Delivery Coordinator Location: Caboolture Office with regular travel across participant locations Employment Type: Full Time As Assist Right Supports continues to grow, we are evolving our leadership structure to better support our participants, our staff, and the future direction of the organisation. We are excited to invite Expressions of Interest from existing team members for the newly created position of Service Delivery Coordinator . This role has been designed to strengthen participant outcomes by providing dedicated leadership across a portfolio of ILO and Community Support participants within a designated geographical area. About the Role The Service Delivery Coordinator is responsible for the day-to-day coordination of participant services, ensuring high-quality, person-centred supports are delivered consistently while maintaining strong relationships with participants, families, support coordinators and other stakeholders. This is a leadership role focused on service delivery, quality and participant outcomes. The role works closely with the Workforce & Performance Coordinator, who will support staff performance, recruitment and workforce development. Key Responsibilities Manage a portfolio of ILO participants and Community Support participants within an allocated geographical area. Lead participant case management, ensuring supports remain person-centred and outcome focused. Develop positive relationships with participants, families, guardians and key stakeholders. Coordinate participant reviews, goal progress and support plan implementation. Ensure services remain compliant with NDIS Practice Standards and Assist Right Supports policies. Monitor incidents, risks and participant trends, implementing proactive strategies where required. Coach and support Support Workers in day-to-day service delivery and best practice. Complete regular quality reviews, documentation audits and participant file reviews. Support new participant onboarding and service commencement. Work collaboratively with the Workforce & Performance Coordinator regarding staffing concerns, performance matters and workforce planning. Contribute to continuous improvement initiatives across the organisation. What We're Looking For We're looking for someone who demonstrates: A genuine passion for delivering outstanding participant outcomes. Strong organisational and time management skills. Excellent communication and stakeholder engagement abilities. Sound understanding of the NDIS Practice Standards and person-centred practice. Ability to prioritise competing demands while maintaining attention to detail. Confidence leading, coaching and supporting Support Workers. A solutions-focused mindset with strong problem-solving skills. Commitment to the Assist Right Supports values of Respect, Integrity, Grit, Honesty and Teaching . Essential Requirements Current Driver Licence. Current NDIS Worker Screening Clearance. Current Working with Children Check (Blue Card) where applicable. Current First Aid and CPR. Experience within disability services, case management or service coordination. Demonstrated leadership or senior support experience.
