Our Mission is to Curate Happiness and Offer Independence to our Participants with our Personalised - Supports. We are seeking an experienced and motivated Support Coordinator to join our team. This Position requires an Individual who has: Experience providing NDIS Support Coordination/Recovery Coaching Services or Similar Role Support participants to understand and implement their NDIS plans Assist participants in building capacity, independence, and decision-making skills Coordinate and connect participants with appropriate service providers Monitor supports and ensure services align with participant goals and funding A Solid understanding of the NDIS and its Importance to People with a Disability and their Support Networks Demonstrated experience with Facilitating Multi-Disciplinary Coordinated Planning and Develop Collaborative Relationships and Networks between Service Providers Maintaining an Up-To-Date knowledge of the NDIS Systems and Practices Engaging with NDIA Staff and LAC’s Liaising with the Participant, Family or Representative on Supporting the Participant with Achieving their Goals Preparing for Plan Reviews and Attending with the Participant, Family or Representative Maintain accurate and compliant documentation in line with NDIS requirements If you or someone you may be enquiring on behalf of on, would like to know further information, please contact us on hr@psupportservices.com.au or call 1300 687 777
