About the role In this role, the Disability Support Coordinator will monitor/implement NDIS-funded supports to help participants achieve wellbeing & community participation goals. Your duties will include but not be limited to: managing daily operations of NDIS Support Coordination Services oversee participant assessments, care planning and referral responses developing collaborative, team-based approaches to participant-centered care partnering with participants to understand needs and create tailored action plans advocating for participant rights and decision-making control promoting independence through social inclusion initiatives. What we're looking for Full Australian working rights essential Min. 2 years previous Disability Support Coordinator experience Qualification in Disability Services, Aged Care or similar Proven NDIS experience with strong operational guideline knowledge WWC Check + National Police Clearance essential First Aid Certificate & NDIS Workers Screening Check essential AUS driver's license + own vehicle and comprehensive car insurance Psychosocial Recover Training desirable .
