Client Engagement & Support  Build trusting, respectful relationships with young people and, where appropriate, families and carers.  Support young people to understand their options and make informed choices about service providers.  Provide trauma-informed, strengths-based, and developmentally appropriate support.  Facilitate warm referrals to alternative services where eligibility is not met or needs fall outside program scope.  Coordinate and attend provider “meet and greet” sessions to support onboarding and engagement. Recovery Planning & Monitoring  Support the development and review of Individualised Recovery and Transition Plans in collaboration with service providers.  Monitor progress against recovery goals and identify emerging support or transition needs.  Contribute to transition planning, including step-down, exit readiness, and connection to natural and community supports.  Maintain accurate, timely, and high-quality documentation within the Client Management System (CMS). Assessment & Administration  Manage referrals across YPSP and YTH&SP streams, including receipt, logging, screening, and acknowledgement. Lived Experience Recovery Coach – Young Person Focus (Peer)  Prepare assessment documentation for panel consideration, including participant summaries, risk considerations, and safety planning.  Assist with convening assessment panels, minute-taking, and drafting outcome correspondence.  Coordinate logistics for panel meetings, provider onboarding, and participant engagement activities. Stakeholder Collaboration  Build and maintain strong relationships with: o Young people and families/carers o Mental health and psychosocial service providers o Housing services and community agencies  Advocate for culturally safe, inclusive, and youth-responsive practice.  Communicate clearly and professionally with stakeholders to support continuity of care and positive participant experiences. Compliance & Professional Development  Comply with the Mental Health Commission service agreement and program guidelines.  Adhere to organisational policies, procedures, and independence safeguards.  Participate in regular professional and reflective supervision.  Attend team meetings, communities of practice, and mandatory training.  Contribute to continuous improvement through feedback, reflective practice, and quality activities.
