Join our Community Living Support team in Penrith. We work with people to build independence and confidence by focusing on their values and personal goals, while encouraging social connection and a sense of belonging in the community. As a Change Facilitator, you will be trained in the Motivational Interviewing change model and will work with clients and stakeholders to assist clients to make significant personal change. With each client, you will map out what is important to them and together develop shared change plans. You will play a key role in building strong relationships, connecting people with community resources, and ensuring high-quality, compliant service delivery through accurate reporting and ongoing review of client progress. Responsibilities Work with each client to understand their needs, co-design personalised plans based on what matters to them, and develop strategies together to manage risks and strengthen wellbeing Monitor, review, and adapt Change Plans in collaboration with clients, mental health services, and stakeholders to support independence and recovery outcomes. Deliver and coordinate individual and group support services that address identified needs and foster community participation. Build and maintain professional relationships with clients and stakeholders, ensuring confidentiality, cultural sensitivity, and strong professional boundaries. Maintain accurate client records and reporting, communicate key updates and risks, and contribute to continuous service improvement and operational efficiency.
