SUPPORT COORDINATOR FULL TIME ROLE About the role NDiCare is seeking a passionate and experienced Support Coordinator to join our dedicated team in Melbourne, VIC. As a Support Coordinator, you will play a vital role in empowering and supporting individuals with disabilities to live independent and fulfilling lives. This is a full-time position that offers a competitive salary and excellent benefits. What you'll be doing Coordinating and overseeing the delivery of individualized support services to clients, ensuring their needs are met Assisting clients in developing and implementing their goals and support plans Liaising with a range of service providers and stakeholders to ensure seamless service delivery Monitoring and reviewing client progress, and making adjustments to support plans as needed Advocating for clients and their families to ensure they have access to the appropriate services and resources Maintaining accurate and up-to-date client records and documentation Participating in team meetings and contributing to the continuous improvement of service delivery What we're looking for Diploma in Disability or Community Services related (desired but not necessary) Minimum 1 year experience in a support coordination or case management role, preferably in the aged care or disability sectors Excellent communication and interpersonal skills, with the ability to build rapport and collaborate effectively with clients, families, and stakeholders Strong organizational and problem-solving skills, with the ability to priorities and manage multiple tasks Demonstrated understanding of the NDIS and aged care systems, and the ability to navigate and coordinate services within these frameworks Empathetic, person-centered approach with a genuine commitment to making a positive difference in the lives of clients Current driver's licence and access to a reliable vehicle For all job enquiries please contact hr@ndicare.com.au Telephone calls to the 1300 NDiCare main line will not be answered.
