You’ll have access to: Participant and practitioner CRM Secure participant management and record keeping Clinical documentation and report templates Service agreement and consent workflows Invoicing and payment systems Compliance framework Professional policies and procedures Clinical supervision or peer support options, where available Administrative support Professional development opportunities Your role As an Independent Support Coordinator, you will: · Manage and grow your own Support Coordination participant caseload · Build and maintain relationships with participants, nominees, providers, Allied Health professionals and community services · Help participants understand and implement their NDIS plans · Connect participants with suitable mainstream, community and funded supports · Support participants to build capacity to coordinate their own supports over time · Assist with service agreements, provider engagement and coordination of supports · Monitor plan implementation, budgets and utilisation within your role · Identify service gaps, barriers and risks and coordinate appropriate responses · Support participants through plan reassessments, change-of-circumstance processes and evidence gathering where required · Maintain clear case notes, records and professional documentation · Manage your own diary, appointments and participant communication · Maintain compliance with the NDIS Code of Conduct, conflict-of-interest requirements and organisational policies · Deliver participant-led, transparent and capacity-building Support Coordination Who we’re looking for We’re seeking support coordinators who are ambitious, independent and committed to delivering high-quality NDIS supports. You will ideally have: Demonstrated experience in NDIS Support Coordination, disability, community services or a closely related field Strong knowledge of the NDIS, participant plans, provider interfaces and service systems Excellent communication, problem-solving and stakeholder engagement skills Strong case management, documentation and time-management skills Ability to identify and appropriately manage conflicts of interest Ability to work independently and manage your own caseload Existing referral pathways or confidence to build your own caseload Relevant qualification in disability, community services, social work, allied health or a related discipline is highly regarded ABN Professional Indemnity and Public Liability Insurance appropriate to your services NDIS Worker Screening Check Working With Children Check where required NDIS Worker Orientation Module Reliable internet, equipment and transport where face-to-face work is undertaken What makes us different?
