About the role Render professional social work services and advice to clients and their families. Identify, plan, initiate and monitor existing and new programs so as to enhance the standard of social work services in Fu Hui Golden Care Centre. Key responsibilities Conduct Initial Assessment, ICCP Performance Reviews with Assistant Centre Manager, Lead Allied Health, Staff Nurse and Physiotherapist Conduct InterRAI CU assessment and implement care plans using InterRAI and Community Care Plan systems Prepare case notes and submission to AIC/MOH Liaise with Next-of-Kin regarding client's financial and social needs Provide casework and counselling services to clients and their NOK Liaise with relevant stakeholders in the preparation of client’s Individual Care Plan (ICP) Work with the Allied Health team and CM as point of contact for caregivers/NOKs Manage feedback, complaints, and service recovery with ACM (when necessary) Plan and execute structured centre-based daily programmes/groupwork including therapeutic, recreational, and social activities Work with the Programme & Services Manager to develop intergenerational and community engagement initiatives Lead the organisation of festive celebrations and key centre events Provide guidance and conduct training relating to Psychosocial topics to the staff team Conduct home visits when necessary About you Degree in Social Work 1 - 3 years' experience in Social Service, Community Care or Healthcare sector (preferred) Knowledge of AIC referral processes (preferred) Basic understanding of dementia care and rehabilitation workflows (preferred) Familiarity with eldercare services (DDC, MDC, rehab) is a plus Proficient in Microsoft Office – MS Word and Excel Knowledge in IT and experience in digitalization is a plus Meticulous and strong coordination and administrative skills Good oral and written communication skills Ability to handle operational issues in a fast-paced environment
