This position will be based at our North Stradbroke Island clinic This is a Full Time Position To provide culturally appropriate support, assessment, and intervention services for individuals experiencing issues related to alcohol and other drugs, they will help assist clients to reduce harm, improve wellbeing, and access treatment, support services and community resources. The role may include outreach, case management, education and advocacy. Alongside clinic and community-based supports to Aboriginal and Torres Strait Islander clients who experience complex social and mental health conditions and to work effectively with other social and health practitioners to identify and access appropriate services to meet their needs. About the position The roles and responsibilities of this position include, but are not limited to: Alcohol and other Drug Worker · Conduct assessments and develop individualized care plans for clients with alcohol and/or drug concerns. · Provide counselling, brief interventions, and therapeutic support to individuals and groups. · Support clients to access detoxification, rehabilitation, mental health, and social services. · Deliver harm reduction strategies and education about substance use, health, and wellbeing. · Collaborate with multidisciplinary teams, including health services, social workers, and community organizations. · Conduct outreach and engagement activities within the community. · Advocate for clients’ needs within the health and social service systems · Participate in program evaluation, development and staff training Case Management · Support the intake and assessment process and being a central point of contact and access for clients · Undertake comprehensive assessment of clients’ psychosocial needs · Direct support to facilitate case management goals. · Assist clients to make informed decisions by acting as their advocate · Conduct home visits or community visits throughout the region to support ongoing client needs. · Manage a caseload of complex clients · Being the main point of contact and access for clients · Liaise and develop external relationships with external services/agencies to collaboratively address client needs. You will, · Have demonstrated ability to work with vulnerable client groups, including the provision of social supports and addressing general client needs. · Have demonstrated knowledge of Aboriginal Community Controlled Health services and the issues facing these organisations. · Have the knowledge or experience in effective case management. · Have the knowledge and experience in effective counselling techniques · The ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people. · Have demonstrated ability to work as part of an inter-professional team · Have demonstrated effective verbal and written skills · The ability to work at YBB clinic at North Stradbroke Island and other YBB sites where directed · Have a Current Blue Card (working with children and young people) Qualifications/Experience · Certificate IV or higher in Alcohol and Other Drugs, Community Services, Social Work, or a related field. · Previous experience in AOD, mental health, or community services is highly desirable. · Understanding of harm reduction, trauma-informed care, and culturally safe practices. · Demonstrated practical experience working with vulnerable clients associated with drug and alcohol or mental health conditions · Desirable Mental Health Accreditation Please attach a one-page cover letter and your resume
