Nurse - Support Services

UPDATED JOB DESCRIPTION SUMMARY: Under the general supervision of the Support Services Director, the Nurse-Support Services is responsible for ensuring that eligible individuals receive quality health and medical services, through either Medicaid Waiver or locally-funded service provision. QUALIFICATIONS: Education/Experience – ·Minimum of 18 months full-time nursing experience (or part-time equivalent), part of which includes providing nursing services to individuals with disabilities Credential/License – · Ohio licensed registered nurse. · Able to obtain an RN Trainer certificate through DODD · Able to obtain a QA RN certification through DODD · Must obtain and maintain First Aid and CPR certification. · American Red Cross Instructor Adult and Pediatric First Aid/CPR/AED certification Other – ·Knowledge of common health conditions and treatment ·Must have reliable transportation. ·Must maintain a valid State of Ohio Driver’s License if operating a personal vehicle for official agency business. PRINCIPAL ACCOUNTABILITIES: Evaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to: Completes Nursing Quality Assurance Assessments and all associated follow-up in accordance with rule. (30%) Provides oversight, monitoring, and guidance regarding health and medical services in residential and day service locations to improve the quality of service delivery by providers in those settings, including certified Medicaid Waiver providers. (14%) Provides consultation and information for Investigative Agents in the course of conducting MUI Investigations. (2%) Identifies potential needs and barriers to quality service delivery and makes recommendations to SSA’s, providers and other team members for individual’s served. (5%) Assists the Support Services Director with developing policy and procedures related to nursing (2%) Participates in stakeholder meetings and work groups and collaborates with county boards, providers and DODD representatives to improve service delivery. (2%) Conducts health-related trainings for certified Medicaid Waiver providers and county board staff, in order to support providers in knowledge and ability to deliver services. (25%) Assists with gathering or providing information for Service Consultants in advance of assessment of medical needs, including those related to Medicaid Services. (5%) Maintains records related to the health and medical needs of individuals served (5%) Reviews and monitors health status of individuals served through documentation and communication with team members and makes recommendations to teams regarding services, care, planning and follow-up (10%) Working Knowledge of all DODD Medicaid Waiver rules and regulations Other duties as assigned.
