Location: North GA Health District Office- Dalton, GA (Hourly Part Time--up to 29 hrs. per week max) Assures Children's First (C1st) program operations include the accurate processing of paperwork, documentation and record keeping of protected health information Conducts home visits to complete comprehensive assessments of clients health care needs Assists (6) counties in District 1-2 in the promotion and assessment of eligible children into the Children First (C1st) Program Develops and sustains community partnerships Works closely with other programs within the Children with Special Needs (CWSN) Unit, such as Children's Medical Services, Early Hearing Detection and Intervention & Babies Can't Wait to ensure linkage to all available services Provided back-up to C1st Referral Coordinator, as needed Screens and refers clients to other community service providers Participates in drills and exercises on mass dispensing Performs job responsibilities with minimal supervision Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification Preferred Qualifications Bilingual-English/Spanish Current State employees are subject to State Personnel Board rules regarding salary. CBH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. All positions require criminal history record check. Required education credentials, license, certification and/or registration must be submitted prior to employment in the position. Some positions require pre-employment drug testing in which the applicant who is offered the position will be required to successfully complete.
