THE IDEAL CANDIDATE The ideal candidate will be organized, detail-oriented, and effective at engaging individuals in a correctional environment. They will be able to review clinical information, interpret assessment data, and develop accurate care plans under clinical oversight. The candidate will be skilled at navigating complex systems and collaborating with the facility medical vendor, Public Health Nurse, Managed Care Plans, and Community Enhanced Case Management Providers. They will also be responsible for maintaining accurate and timely documentation that supports all fee-for-service billing activities, including tracking required service components, documenting billable encounters, and ensuring all information meets DHCS CalAIM Justice-Involved standards. THE POSITION The Senior Community Health Worker provides person-centered care coordination services to justice-involved Medi-Cal beneficiaries during the 90-day pre-release period under the CalAIM Justice-Involved Reentry initiative. The SCHW works inside the Correctional Facilities and collaborates closely with the medical vendor, correctional staff, MCP care managers, ECM community providers, and reentry partners to support a transition back into the community. The SCHW plays a central role in initiating care coordination, supporting Medi-Cal processes, and helping individuals successfully access services post-release. The Senior Community Health Worker is responsible for accessing the Medical Vendor’s electronic health record (EHR) to review all completed clinical assessments performed by the Medical Vendor, including, but not limited to, mental health initial assessments, intake assessments, and associated diagnostic or treatment documentation. The SCHW analyzes the information obtained through these assessments to develop individualized and clinically appropriate Reentry Care Plans. This work is conducted in close collaboration with, and under the oversight of, the Public Health Nurse (PHN), who provides clinical guidance to ensure accuracy, regulatory compliance, and alignment with CalAIM Justice-Involved requirements. Through this process, the SCHW coordinates release-related needs and supports continuity of care between the correctional setting, Managed Care Plans, and ECM community providers. This is the advanced working level in the Community Health Worker series. Job assignments require the ability to provide service with minimal guidance and supervision. Positions at this level are distinguished from the Community Health Worker level by performance of the full range of duties as assigned, working independently, and exercising judgment and initiative. Duties may include, but are not limited to, the following: Interviews clients to document health conditions and determine eligibility for program participation; Assists in interviewing, gathering and assembling related information and preparing reports; maintains appropriate records and files for clients/patients; Obtains medical histories, records observations, maintains clients' records; Educate and re-enforce treatment plans prescribed by medical providers Organizes data, develop information, and prepare clear, concise, comprehensive records and reports. Assist clients with referrals to services and resources; May assist with a variety of concerns impacting clients/families, including but not limited to the control and prevention of chronic condition or infectious diseases, behavioral health conditions, and need for preventive services; Develops individualized Reentry Care Plans with Public Health Nurse oversight, ensuring plans reflect medical vendor recommendations and reentry requirements; Updates care plans based on new information, clinical changes, or updated release timelines; Documents all care coordination activities accurately in required systems; Works closely with the medical vendor to ensure DME needs are identified and secured before release, medication needs and 30-day post-release supplies are prepared, clinical recommendations are incorporated into care plans, information required for warm handoffs is complete and accurate; Ensures clinical information is collected for reentry planning; Facilitates warm handoffs to community ECM providers; Ensures timely communication and clear documentation or all care coordination; Coordinates post-release appointments (primary care, specialty care, behavioral health, SUD treatment); Works with the medical vendor to ensure individuals are released with medications, prescriptions, and DME; Ensures consent is obtained for communication with MCPs and community providers; Participates in case reviews, care coordination meetings, and reentry planning discussions; Provides program updates and maintains strong communication with internal and external partners; Documents all care coordination activities in alignment with fee-for-service billing requirements, including required service elements, billable components, and supporting narrative detail; Participates in quality improvement efforts, audits, and regulatory reviews by ensuring documentation is complete, organized, and audit-ready; Maintains strict confidentiality of protected health information and adheres to HIPAA, facility security protocols, and all applicable privacy regulations; Assists with meeting coordination, communication, and workflow support; Maintains program materials, trackers, and data logs; and Works independently and exercises sound judgment. EDUCATION/EXPERIENCE PATTERN I Equivalent of twelve (12) months of clerical and paraprofessional nursing training or experience, either voluntary or paid, in a health care setting; AND Fifteen (15) related semester units of college-level course work, including at least one class in either: Psychology, Sociology, Biology or Physiology; AND One (1) year of experience comparable to a Community Health Worker in Stanislaus County. PATTERN II Two (2) years of experience comparable to a Community Health Worker in Stanislaus County. PATTERN III Equivalent of twelve (12) months of clerical and public health training or experience, either voluntary or paid, in a public health department; AND Fifteen (15) related semester units of college-level coursework, including at least one class in either: Behavioral Science, Biology, Social Services, Nutrition, Health Education or a closely related field. AND One(1) year of experience comparable to a Community Health Worker in Stanislaus County. LICENSE/CERTIFICATION Some positions may require completion of a Community Health Worker certification. If the selected candidate does not have this certification, they must begin certification training within six-months of employment.
