As a Sr Software Engineer /PHP Developer on the Expert Medical Opinion (EMO) platform, you will help design, build, and operate the technology that connects members with world-class medical experts when they need them most. This is a Prague-based position with a hybrid working model — we expect you in the office 2–3 days per week, collaborating with the local Prague team and our colleagues in the US. You’ll work on a platform that blends a long-running PHP codebase with a modern Domain-Driven Design layer, deployed on AWS. You’ll partner with product managers, internal stakeholders, and your manager to turn complex healthcare requirements into reliable, well-architected software — and have a direct impact on the quality of care our members receive. What you’ll do: Understand business requirements and the scope of enhancements, working closely with product and stakeholders Design, architect, and plan the delivery of new features and ongoing maintenance with a clear systems-design mindset Take ownership of deliverables, hitting timelines without compromising quality Write and maintain solid Unit and Functional tests as a standard part of how you ship Perform thoughtful code reviews and provide coaching and mentorship to more junior team members Use AI coding tools effectively — with a strong focus on architecture, review, and rigorous validation of AI-generated solutions rather than blind acceptance Help evolve our existing PHP codebase — layering in DDD, strengthening boundaries, and gradually moving toward a cleaner, service-oriented architecture What we’re looking for: Solid experience developing backend web applications with MySQL, alongside server-rendered HTML / CSS / JavaScript — our frontend is not a modern SPA, though we do have some components built in Vue.js; comfortable working with PHP (prior PHP experience is a plus, not a hard requirement) Strong discipline around automated testing — writing and maintaining Unit and Functional tests is a must, not an afterthought Solid grasp of layered architecture and separation of concerns (e.g. Domain-Driven Design, Clean/ Hexagonal architecture) — able to reason about boundaries, dependencies, and where logic belongs Experience in Cloud Architecture or a strong systems-design mindset — comfortable reasoning about scalability, reliability, and trade-offs Experience working on Enterprise applications — or a strong appetite to dive into a complex business domain (healthcare / medical consultations) and grow into a domain owner over time AI-friendly developer — comfortable working alongside AI coding tools, with a focus on architecture and rigorous validation of AI-generated solutions Working experience building REST APIs and using Git in a collaborative team setting Solid foundation in modern DevOps culture and comfort with Docker-based development workflows Skillful communicator who thrives in a collaborative, agile environment and works well with technical and product stakeholders Nice to have: Prior experience with PHP and frameworks such as CodeIgniter or Symfony Hands-on experience with AWS services such as IAM, S3, RDS, ECS, and API Gateway Experience working in healthcare, regulated industries, or other domains with strong privacy/security requirements Experience modernizing or migrating legacy codebases toward microservices or service-oriented architectures Familiarity with static analysis tools (e.g. PHPStan, Psalm), architectural fitness tooling (e.g. Deptrac), or similar quality-enforcing tooling. If you just finished reading one job ad and felt paralyzed by choice, you're not alone—but this scale is actually an advantage. It means you can afford to be selective.

Start by geography and language. The majority of engineer ads—117,837 of them—have the job posting text written in English. Use that as one filter, but remember: the ad text language tells you nothing about whether the role actually requires you to speak English day-to-day. Read the job description carefully. Then check which countries have the volume you're targeting. Singapore, Poland, and Australia round out the top five after India and the US.
