The Community Health Registered Nurse I focuses on prevention, health restoration, maintenance or palliation in the delivery of care/coordination of service to clients, designated care givers or families. The Community Health Registered Nurse I will ensure person and family centered care that wraps available resources around the individual wherever the individual resides. The Community Health Registered Nurse I will establish a preventative, integrated, person and family centered approach driven by individualized assessment of needs and individual choice to promote aging in place.The Community Health Registered Nurse I will practice utilizing the Home First philosophy in identifying, assessing and supporting individuals with complex care needs to be supported in their own home. Work is performed in accordance with the Canadian Community Health Nursing Standards of Practice and within the policies and procedures of the Central Regional Health Authority. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.Required qualifications:? Graduation from an approved/recognized post-secondary Nursing Education program with an undergraduate degree in Nursing or Registered Nurse with an approved Diploma in Community Health Nursing.? Current registration with the Association of Registered Nurses of Newfoundland and Labrador.? Three (3) years clinical experience.? One (1) year experience within the last three (3) years providing and coordinating community health nursing service.? Current BLS certification for health care providers from the Heart and Stroke Foundation.? Current Health Assessment certification or willingness to complete the next available course.? Demonstrated exceptional leadership qualities and problem-solving skills, as well as, oral and written communication skills.? Demonstrated ability to work well independently and with others in an interdisciplinary team environment.? Demonstrated initiative.? Willing to work a flexible work schedule.? A valid Newfoundland and Labrador Class IV Driver?s License and the use of a private vehicle are conditions of employment.?
