This role involves (but is not limited to):Coordinating patient care by assessing, planning, evaluating and discussing/reviewing with the physician.Supporting patients with complex care navigation and care coordination, helping them access appropriate healthcare and community resources.Working collaboratively with physicians to carry out delegated clinical tasks and responsibilities within scope and competency.Performing primary care procedures within the Registered Nurse scope of practice.Contributing to the delivery of immunizations, well baby care, pap tests, and patient assessments for a variety of health concerns.Maintaining patient files, encounter information, clinical data, and other information in a careful and confidential manner via EMR Software.Answering inquiries in a helpful, respectful and efficient manner.Reviewing and processing faxes electronically, including patient referrals.Educating, advising, and empowering patients on preventive healthcare and disease prevention.Liaising between clinic, hospital, specialists, home care and community agencies to ensure integrated, continuous care.Staying up to date with medical advancements to provide high-quality care.Performing all other related clinical, administrative or special tasks as needed.Experience & Education Required:The applicant must have successfully completed a Bachelor of Science in Nursing.The applicant must be currently registered and in good standing with the Nurses Association of New Brunswick (NANB).Previous experience across various areas of medicine, particularly in primary care or community health is considered an asset.Basic CPR certification.Advanced health assessment and clinical skills combined with sound judgement relevant to dealing with a wide variety of ages and health concerns.Knowledge and proficiency in current, evidence-based methods and practices for primary care delivery, with an emphasis on health promotion, risk reduction and chronic disease management.Experience using an electronic medical record software (EMR) is an asset.General knowledge of the New Brunswick medical system is an asset.Qualifications:Demonstrates advanced clinical and critical thinking.Ability to maintain a high regard for confidentiality and advocacy for patients.Ability to multitask and prioritize effectively in Location Moncton , NB Work location On site Salary $ 45.00 to $ 52.00 HOUR hourly Terms of employment Permanent employment Full time Starts as soon as possible vacancies 1 vacancy Source CareerBeacon #MB2608207558 Various locations Moncton, NB
