Sanctum is a program that provides transition, respite, and palliative care to HIV positive individuals. The home support worker will provide care and support to residents with HIV, who may also be affected by mental illness, addictions, poverty, homeless or other psycho-social factors. The home support worker will embody a patient centered harm reduction care model fostering an environment of acceptance and understanding for clients. This position is for all shifts, weekdays and weekendsDays: 07:30 - 16:00Evenings: 15:30 - 00:00.Nights: 23:30 - 08:00 ResponsibilitiesPersonal care & hygieneAssist with and support clients with medication adherence.Establish and maintain current, accurate, confidential filesfor each resident.Follow medical instructions carefully and consistently.Assist with meal preparation and planning.Household duties such as cleaning and maintenanceManage any unanticipated events or unstable situations. QualificationsHarm Reduction & Trauma and Violence Informed Care.
