*The Brockton and Area Family Health Team* Is inviting applications for a *Health Educator* *Temporary Part-Time Position, 22.5 hours per week* *Chesley Site* The successful candidate will play an integral role in providing care to patients of the Brockton and Area Family Health Team (BAFHT) at the Chesley Site. The Health Educator will work collaboratively with physicians, allied health professionals, and administrative staff and will demonstrate strong interpersonal and communication skills in supporting patients and their families. *Primary Responsibilities* · Provide health education and support to help patients improve or maintain their health and develop self-management skills in areas such as diabetes, hypertension, geriatric health, and smoking cessation. · Develop and deliver health education programs for patients and community members. · Assist patients in identifying and accessing appropriate health resources, including community services and agencies. · Engage patients in informed decision-making regarding their health and care. · Support patients in developing effective self-management strategies. · Lead and contribute to health promotion and health education initiatives. *Position Qualifications * * Registration with the College of Nurses of Ontario. * Minimum of three (3) years of nursing experience. * Previous experience in community health or primary care. * Excellent oral and written communication skills. * Demonstrated ability to work effectively within an interdisciplinary team. * Demonstrated knowledge and experience in health promotion and chronic disease prevention. * Proven ability to communicate effectively with patients, families, and colleagues. * Experience coordinating patient care and connecting patients with community resources. * Proficiency with computers and information technology. * Valid driver?s license and access to a reliable vehicle.
