The Hospital Liaison plays a key role in increasing access to culturally appropriate healthcare services and supporting Indigenous clients and families in navigating the healthcare system. Working collaboratively with the Saskatchewan Health Authority, Non-Insured Health Benefits (NIHB), community partners, and BRT6HC programs, the Hospital Liaison helps ensure clients receive wholistic, coordinated, and culturally safe care throughout their healthcare journey. The position provides advocacy, health system navigation, discharge planning, and client support services while fostering communication and collaboration between hospitals, community-based programs, and family support networks. Through relationship-building and a client-centered approach, the Hospital Liaison promotes positive health outcomes and enhances the overall patient experience. Licensed Practical Nurse (LPN) is in good standing with CLPNS. Minimum three (3) years of community health nursing experience; experience working in a First Nations community is considered an asset. Demonstrated understanding of the health and social issues affecting First Nations people and communities. Knowledge of population health principles, health promotion, and disease prevention concepts. Experience working with First Nations clients and families, with a commitment to cultural safety, humility, and ongoing learning about First Nations cultures, traditions, and perspectives. Ability to work effectively both independently and as part of an interdisciplinary team in a self-motivated and professional manner. Strong advocacy, problem-solving, and client-centered care skills. Excellent interpersonal, communication, and relationship-building skills. Excellent written and verbal communication skills, including documentation and reporting abilities. Proficient computer skills, including experience with Microsoft Office applications and electronic documentation systems.
