The File Hills QuAppelle Tribal Council is a dynamic and progressive organization that provides a range of programs and health services which serve eleven First Nations throughout Southern Saskatchewan within the Treaty Four Territory. Based out of the Pasikow Muskwa Rising Bear Healing Centre, All Nations Healing Hospital the Health Navigator will work as part of a multi-disciplinary health care team providing chronic disease and enhanced kidney care services to meet client care needs within a holistic model. Reporting to the Director of Acute & Clinical Services or designate, the successful candidate will support clients, families, and communities in accessing healthcare services while incorporating traditional, cultural, and holistic approaches to wellness. The Health Navigator will enhance patient experience by;Advocating for patients, families, and communities Supporting access to traditional and cultural services in collaboration with the First Nations Cultural Practitioners Bridging communication between healthcare providers, community agencies, and clients Coordinating care transitions and supporting clients throughout their care journey Providing education, counseling, and resource navigation services.The successful candidate will be expected to maintain flexibility in service delivery and may provide Health Navigator support across programs, departments, and service areas within the organization to ensure continuity of care and meet operational requirements.Applicants must possess: Recognized health degree (i.e. Social Work, Nursing, M.A. Psychology) Current practicing registration with appropriate licensing body Valid Saskatchewan Drivers LicenseQualifications: Must be considered to be respectful, approachable and supportive with other agencies to work together in a common goal to wellness Must have understanding and knowledge of programs and services provided in regional health authority, Health Canada, intersectoral services Location Fort Qu'Appelle , SK Work location On site Salary $ 72,147 to $ 86,508 YEAR annually Terms of employment Full time Starts as soon as possible Source SaskJobs #1538778 Various locations Fort Qu'Appelle, SK
