POSITION SUMMARY: St. Mary’s Health @Home is dedicated to providing quality, compassionate, and client centered care to individuals in their homes and/or alternative settings through our various programs. Registered Nurses enhance the life of their clients by providing a broad range of care needs within the framework, policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario. Through their work, Registered Nurses contribute to the healing and independence of their clients and promote the continual improvement of the overall health of the community. CORE DUTIES AND RESPONSIBILITIES utilize the nursing process and standards of care (College of Nurses of Ontario – RN) perform pronouncements in keeping with RN standards monitor and comply with DRN documentation work independently in the community in the execution of daily tasks complete point of care assessments and comprehensive nursing assessments (e. g. Mini-metal status, depression, pain, etc.) practice infection prevention and control measures engage as a support network for peers also working in the community provide the highest quality care and participate in quality processes as required complete all necessary client assessments implement and deliver client’s plan of care, recommend care plan adjustments to Managers participate in the effective and safe coordination of client care among team members and managers ensure appropriate caseload management, ongoing monitoring and evaluation of care. collaborate respectfully with the client, family members, other team members engage other care multi-disciplinary providers as necessary (e. g.
