As a Licensed Practical/Vocational Nurse with Caring Hand Home Health Solutions, you will play a meaningful role in helping patients heal, stay safe, and thrive in the comfort of their own environment.We value teamwork, empathy, and a commitment to high-quality service, all aimed at putting patients and their families at ease during their recovery. If you are passionate about making a positive impact in patients' lives and working in a supportive and family-oriented atmosphere, we encourage you to join us in providing exceptional home health care.Please note that this role is per diem with a rate of $45 - $55 per visit.ResponsibilitiesProvide direct patient care in accordance with the state Nurse Practice Act.Implement and follow individualized care plans established by the RN.Educate patients and their families on self-care techniques and diagnosis-related information.Document care accurately and in a timely manner.Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.Use supplies and equipment safely, adhering to aseptic technique.Perform other duties as required for patient services.Graduate of an accredited practical or vocational nursing program.Active LPN or LVN license in California.3+ years of nursing experience; home health or med/surg background preferred.Current CPR certification with AHA.Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.Strong communication skills and a collaborative, patient-first mindset.Flexible ScheduleAbility to Convert to Full-Time Based On Performance If InterestedPaid Sick DaysPaid Bereavement LeaveSkills Training & DevelopmentCompany Social EventsMileage Reimbursement for Patient to Patient VisitsReferral Bonus #J-*-Ljbffr
